you need to specify the location of the SuperPuTTy and the protocol to use as below:
"C:\Program Files (x86)\SuperPuTTY\SuperPutty.exe" -proto -telnet "%h -P %p"
After that, go and open SuperPuTTy application and set the following settings:
Select Only allow single instance of SuperPuTTy to run as you see in the above picture.
and on GUI tab, under tabs and docking set tab text to dynamic , as you see below on the picture.
